Not every wisdom tooth requires removal. When there is sufficient space in the jaw and the tooth erupts cleanly into a functional position, it can simply be monitored at routine check-ups like any other tooth. The trouble arises when space runs out and the tooth cannot complete its journey — that is when symptoms typically begin and clinical intervention becomes necessary.

The four most common situations that lead us to recommend extraction are: first, when the tooth is clearly unable to emerge into a useful position and is causing persistent pain or discomfort; second, when the tooth has partially erupted and decay has taken hold, as partially exposed wisdom teeth are notoriously difficult to clean effectively; third, when the tooth is creating serious oral hygiene problems despite having no functional value; and fourth, when the tooth has overgrown due to a missing or impacted opposing tooth and is now causing bite or jaw problems.

Do wisdom teeth always need to be removed?

No — and we will never recommend removal unless the clinical evidence supports it. If your wisdom tooth has space to erupt into a healthy, functional position without causing problems, we simply monitor it at routine appointments. Extraction is only advised when the tooth is impacted, infected, decayed, or actively damaging neighbouring teeth or gum tissue.

Common signs include persistent pain at the back of the jaw, swollen or bleeding gums in that area, difficulty fully opening your mouth, a recurring bad taste or infection at the back of the mouth, and a tooth that has only partially appeared through the gum. A clinical examination and X-ray is the only reliable way to confirm impaction — which we can arrange at our Romford Road practice.

The procedure is carried out under local anaesthesia, so you will feel pressure but no sharp pain during removal. If at any point you feel discomfort, tell your dentist and they will pause immediately. Beforehand, if you are in pain, warm saltwater rinses, Corsodyl mouthwash, and paracetamol can help manage symptoms. Avoid aspirin as it can increase bleeding — and asthmatics should avoid ibuprofen.

Most patients experience some swelling and discomfort for a few days post-procedure — this is a normal healing response. Avoid rinsing, alcohol, hot food, and straws for the first 24 hours to protect the blood clot. After 24 hours, rinse gently with warm saltwater twice daily for at least a week. Rest quietly for the first day and follow all aftercare instructions provided at your appointment.

Treatment is available on the NHS and privately at our practice. NHS charges apply unless you qualify for exemption — patients under 18 and those eligible for NHS exemption are treated free of charge. Hospital treatment under general anaesthesia is provided free through the NHS. Private treatment costs vary by complexity. We always discuss fees and available payment options — including 0% finance on selected treatments — before beginning any procedure.
